The Resident Navigation Specialist serves as a guide into programs and services within and beyond CAPP. We assist income eligible residents achieve their goals and move towards self-sufficiency through:

• Comprehensive assessments that identify individual strengths and barriers
• Internal and external referrals
• Continuous monitoring and evaluation of service delivery
• Modification of action plans, as needed
